Universal Service Fund E-Rate Participant

DSL Forms, Agreements, and Contracts

In order to receive DSL service through Internet Nebraska, some additional forms need to be filled out in hardcopy. If you sign up via the DSL signup page, you will not need to download and print these documents.

One agreement with Internet Nebraska needs to be signed and returned to our office, and if you are signing a term contract (eg. to get the free router promotion offer when and where available) you will need to also fill out and return Windstream's DSL contract.

In the unfortunate event that DSL service is not available in your area, you will be notified, and your application will be kept on file. When DSL service is available in your area, you will be notified again, and asked if you would still like to sign up for DSL access at that time.

It can take approximately 2 weeks from the time DSL service is ordered to the time Windstream is able to install the DSL service on your phone line. We will let you know as soon as we have more information on the state of your DSL application. Please direct all DSL status questions to
